Can be found in high tier loot crates as well as from boss enemies. Once scanned an objective is placed on the map and a quest is generated. Map icon will disappear once you get close to the objective so it is considered best practice to zoom in on the icon and place your own marker in the center of it. Not all icons have the objective at ground level.
